Members of The Network: Towards Unity for Health, should NOT use the online registration but print and use this PDF registration form. Once printed and completed, it can be sent by fax to the number provided on the registration form.
Conference fees include: admission to all sessions, documentation (handed out at the registration desk),
lunches and refreshments and accommodation on the 15 and 16th of September including breakfast.
The additional fee for accommodation (incl. breakfast) on Sunday, September 14th is
€ 50.00 and must be paid in cash on the first day of the conference. The deadline for Sunday room reservations is THURSDAY, AUGUST 28TH .
